The assignmentfocused soap note for anxiety, ptsd | NUNP 6670 – Psychiatric Mental Health Nurse Practitioner Role II: Adult and Older Adult | Walden University

In assessing patients with anxiety, obsessive-compulsive, and trauma and stressor-related disorders, you will continue the practice of looking to understand chief symptomology in order to develop a diagnosis. With a differential diagnosis in mind, you can then move to a treatment and follow-up plan that may involve both psychopharmacologic and psychotherapeutic approaches.

In this Assignment, you use a case study to develop a focused SOAP note based on evidence-based approaches.

The components of the SOAP note are;

  1. Subjective: This section includes information provided by the patient or their family members regarding their symptoms, concerns, and overall subjective experience. It encompasses the patient’s description of their condition, any changes since the last visit, and other relevant subjective information.
  2. Objective: The objective section consists of measurable and observable data gathered by the healthcare professional. This may include vital signs (e.g., blood pressure, heart rate), physical examination findings, laboratory or diagnostic test results, and any other objective data relevant to the patient’s condition.
  3. Assessment: In the assessment section, the healthcare professional provides their clinical judgment or assessment of the patient’s current condition. This includes analyzing the subjective and objective information to form a professional opinion about the patient’s health status, potential diagnoses, and any changes or progress observed.
  4. Plan: The plan section outlines the proposed or ongoing management plan for the patient. It includes the healthcare provider’s recommendations, interventions, treatments, medications, referrals, and follow-up plans. The plan is typically focused on addressing the patient’s needs and improving their health outcomes.
