Create \u00a0\u00a0\u00a0\u00a0\u00a0a focused SOAP note for this patient using the template in the \u00a0\u00a0\u00a0\u00a0\u00a0Resources.<\/li>\n<li>Your presentation should \u00a0\u00a0\u00a0\u00a0\u00a0include objectives for your audience, at least three possible discussion \u00a0\u00a0\u00a0\u00a0\u00a0questions\/prompts for your classmates to respond to, and at least five \u00a0\u00a0\u00a0\u00a0\u00a0scholarly resources from 2018-2022 to support your diagnostic \u00a0\u00a0\u00a0\u00a0\u00a0reasoning and treatment plan.<\/li>\n<li><strong>Be succinct in your presentation, \u00a0\u00a0\u00a0\u00a0\u00a0and specifically address the following for the patient, using your SOAP \u00a0\u00a0\u00a0\u00a0\u00a0note as a guide.<\/strong>\n<ul>\n<li><strong>Subjective: <\/strong>What details did the \u00a0\u00a0\u00a0\u00a0\u00a0\u00a0patient provide regarding their chief complaint and symptomology to \u00a0\u00a0\u00a0\u00a0\u00a0\u00a0derive your differential diagnosis? What is the duration and severity of \u00a0\u00a0\u00a0\u00a0\u00a0\u00a0their symptoms? How are their symptoms impacting their functioning in \u00a0\u00a0\u00a0\u00a0\u00a0\u00a0life?<\/li>\n<li><strong>Objective: <\/strong>What observations did you \u00a0\u00a0\u00a0\u00a0\u00a0\u00a0make during the psychiatric assessment?<\/li>\n<li><strong>Assessment:<\/strong> Discuss their mental \u00a0\u00a0\u00a0\u00a0\u00a0\u00a0status examination results. What were your differential diagnoses? \u00a0\u00a0\u00a0\u00a0\u00a0\u00a0Provide a minimum of three possible diagnoses and why you chose them. \u00a0\u00a0\u00a0\u00a0\u00a0\u00a0List them from highest priority to lowest priority. What was your primary \u00a0\u00a0\u00a0\u00a0\u00a0\u00a0diagnosis, and why? Describe how your primary diagnosis aligns with <em>DSM-5-TR<\/em> diagnostic \u00a0\u00a0\u00a0\u00a0\u00a0\u00a0criteria and is supported by the patient\u2019s symptoms.<\/li>\n<li><strong>Plan:<\/strong> What was your plan \u00a0\u00a0\u00a0\u00a0\u00a0\u00a0for psychotherapy (include one health promotion activity and patient \u00a0\u00a0\u00a0\u00a0\u00a0\u00a0education)? What was your plan for treatment and management, including \u00a0\u00a0\u00a0\u00a0\u00a0\u00a0alternative therapies? Include pharmacologic and nonpharmacologic \u00a0\u00a0\u00a0\u00a0\u00a0\u00a0treatments, alternative therapies, and follow-up parameters, as well as a \u00a0\u00a0\u00a0\u00a0\u00a0\u00a0rationale for this treatment and management plan. \u00a0Discuss an \u00a0\u00a0\u00a0\u00a0\u00a0\u00a0identified social determinate of health impacting this patient\u2019s mental \u00a0\u00a0\u00a0\u00a0\u00a0\u00a0health status and provide your recommendation for a referral to assist \u00a0\u00a0\u00a0\u00a0\u00a0\u00a0this patient in meeting this identified need (students will need to \u00a0\u00a0\u00a0\u00a0\u00a0\u00a0conduct research on this topic both in the literature and for community \u00a0\u00a0\u00a0\u00a0\u00a0\u00a0resources).<\/li>\n<li><strong>Reflection notes:<\/strong> What would you do \u00a0\u00a0\u00a0\u00a0\u00a0\u00a0differently with this patient if you could conduct the session \u00a0\u00a0\u00a0\u00a0\u00a0\u00a0again?
