Task<\/p>\n

In order to meet the complexities of modern healthcare, you are required to enter the healthcare workforce with the skills, knowledge, mindset and behaviours for becoming a nurse\/nursing associate.<\/p>\n

Assessment of evidence-based, person-centred care needs is an important skill in developing your nursing practice.<\/p>\n

The person you select to base the reflection and assignment on should come from your area of practice: learning disability, mental health, children\u2019s or adult nursing.<\/p>\n

The risk assessment that you use should be developed with the person you have selected and should also involve their carers and\/or parents (where appropriate) and\/or the multi-disciplinary team (where appropriate).<\/p>\n

Remember that confidentiality is important and avoid including information that identifies the Trust, ward or person. Use pseudonyms.<\/p>\n

  1. Reflect on your learning from TMA 01 and TMA 02 and identify with your Practice Supervisor an individual for whom you have had an opportunity to deliver person-centred care under the indirect supervision of your Practice Supervisor.<\/li>\n
  2. Select an assessment tool relevant to your area of practice and to the individual for whom you will delivering person-centred care, and consider the individual\u2019s care plan in partnership with them. Use the assessment tool and individual care plan when preparing to complete your EMA assignment.<\/li>\n
  3. Select ONE care need that you have identified from the assessment tool and ONE care need you have identified from the care plan in discussion with the person involved.<\/li>\n
  4. Use a reflective tool of your choice to demonstrate how you have developed personally and professionally during your practice learning experience, drawing on the TWO care needs you have identified in point 3.\u00a0(1500 words)<\/b><\/li>\n
  5. Update your personal development plan, reflecting on the skills you have identified that require further development.\u00a0(500 words)<\/b><\/li>\n
  6. Submit the completed EMA along with Appendix 1, which should include:\n
