You will continue to work in your groups in order to write a grant proposal. Based on the <\/span>
template provided in Chapter 14, you will request funding for the practice initiative developed <\/span>
over three weeks. Be sure to be persuasive and use citations from the literature to back up why <\/span>
your initiative will work. <\/span>
I<\/span>NSTRUCTIONS<\/span>
Include at least 4 peer-reviewed referenced in addition to the course textbook and the Bible.<\/span>
The grant proposal must include the following: (see Chapter 14 of the textbook for the <\/span>
components of a proposal)<\/span>
\uf0b7<\/span> Executive summary (1 page)<\/span>
\uf0b7<\/span> Statement of need (1 \u2013 2 pages): include target population, purpose, goals, and <\/span>
significance of initiative (what defines success)<\/span>
\uf0b7<\/span> Brief overview of organization (1 page): this refers to the original organization that the <\/span>
practice initiative was linked to during Module 3: Week 3 (e.g. Cleveland Clinic, etc.).<\/span>
\uf0b7<\/span> Project description (2 \u2013 4 pages): problem statement, describe project, statement of the <\/span>
problem, how the project will meet the need(s), procedures, resources, <\/span>
deliverables\/projected outcomes, evaluation plan, and dissemination plan. <\/span>
\uf0b7<\/span> Outcomes\/Impact (1 page): discuss the anticipated outcomes\/impact of the project and <\/span>
how the funds will be used to attain the outcomes. Outcomes must be both measurable <\/span>
and concise.<\/span>
\uf0b7<\/span> Sustainability (1 \u2013 2 pages): Discuss any \u201cin-kind\u201d funding or what percentage of funding <\/span>
will be required from the grant funder. Discuss how the program will be sustained once <\/span>
the grant funds are exhausted.<\/span>
\uf0b7<\/span> Budget narrative (1 page) List budget categories and amounts. This includes a clear <\/span>
listing of the financial amount and a clear and concise description of the resources <\/span>
(including personnel).<\/span>
