Frank works for Northern rail as a ticket inspector. Belinda works part time as a medical secretary at a GP practice. They have threen teen age children, Janine (age 15), Joselyn (age 13) and Alfred (age 11). Since the death of Belinda’s father in 2018, Belinda’s mother, Joy (age 78) has been living with the family.

Both Frank and Belinda’s parents were windrush generation migrants from Trinidad and Tobago. Frank and Belinda describe their ethnicity as Black British. They met in their local neighbourhood of Tottenham, North London where they both attended the local church and settled in Liconlnshire before the three children were born, when Frank was offered a new position in the railway industry. Frank had wanted to leave London as both he and Belinda hoped to have a family and could not see how they could do this whilst living in a small flat. They felt that the move out of London would offer them the chance to purchase a property and jobs that would improve their opportunities. Frank’s parents, who are becoming increasingly frail and dependent, still live in London, where they are increasingily supported by Frank’s younger sisters, Gloria and Grace.

When Alfred was born, there were perinatal complications from his premature birth at 28 weeks that eventually resulted in a diagnosis of celebral palsy (hereafter referred to as CP). Long term health conditions- Joy has scored 12 on the Edmonton Frail Scale (EFS). She mobilises with a rollatore frame. She has incontinence of urine, has some memory difficulties and macular denegeration resulting in visual impairment. She can managed one or two stes so long as there is a grab rail in place.

Frank has a body mass index (BMI) of 28.4 and has type two diabetes. He works in a sedentary job and feels he does not have the time to exercise regularly.

Belinda has a BMI of 27.3 and has started to have aches in her joints when she moves- especially in the morning. Her doctor has told her she is likely to have osteoarthritic changes in her hips and knees. This not only makes her very uncomfortable but also less inclined to take any exercise or walk too far. Alfred mobiles with indoor and outdoor using manual self-propelled wheel chair due to spasticity/high tone in all four limbs. He can weight bear and perform transfers with supervision of one person. He has a learning disability which is described as mild. His speech is sligtly affected by the CP but he can make himself understood. Given his health condition, Alfred is known to the local authority children with disabilities team and he is on voluntary children disability register (Children’s Act 1989, s17) (https: www.Lincolnshire.gov.uk/special educational needs-disabilities-send/join-children’s-disability register. Given the pressure on the family, Alfred’s social worker has also made a referral to the local authority social care team to access Joy’s needs and to review Frank and Belinda’s needs as carers of both Alfred and Joy (under the care act 2014).

Socio-cultural factors

Frank and Belinda are regular attendees of the evalengical church. They are one of a few BAME families in the neighbourhood. They both enjoy gardening and maintain a small vegetable plot in their back garden. Frank loves to cook and Belinda is a keen seamstress. She made all the curtains in the house and enjoys making her own clothes. They have a family car which Frank drives. Belinda does not drive. She tends to use public transport or taxis to get around while Frank is at work.

School

All three children attend the same local state school. Alfred is in his first year of secondary education and is now well supported by his statement of educational needs and support package. The school is full accessible for wheelchair users and Alfred is provided with a taxi to school and back.

You are required to write a 2500 word essay that shows your understanding of the module learning outcomes 2,3, and 4.
