Module: Week under <\/span>Center for Disease Control and Prevention: Epi Info<\/span>TM<\/span> Once <\/span>
downloaded to your computer, there are numerous YouTube videos and tutorials that teach this <\/span>
program but perhaps the most helpful resource is the <\/span>Epi Info 7 User Guide<\/span>. It may prove <\/span>
beneficial to print off a copy of the portions you need to refer to while you are using Epi Info 7.2. <\/span>
After learning this wonderful program, develop an outbreak survey from the fictitious disease <\/span>
provided for you below, using the Epi Info 7.2 software. Submit your assignment with cover <\/span>
page. Include in this assignment the topic you have picked for your final outbreak paper and your <\/span>
rationale for choosing that topic.<\/span>

I<\/span>NSTRUCTIONS<\/span>
Allow yourself adequate time to navigate and learn this program. It will take substantial time <\/span>
(several days) to complete! You do not have a final comprehensive examination to study for in <\/span>
this course so consider this the most time-consuming portion of the course.<\/span>
Fictitious disease scenario for development of survey questionnaire:<\/span>
A graduation party was held in Springfield, VA for students and parents from the senior class at<\/span>
Springfield Christian Academy. <\/span>
The following meal was catered:<\/span>

\uf0b7<\/span> Tossed green salad with optional blue cheese, oil and vinegar, buttermilk, or thousand <\/span>
island dressings<\/span>

Twenty-four hours after the party, members of the class and family members began to get ill. <\/span>
They experienced various severity of intestinal disorders including nausea, vomiting, diarrhea, <\/span>
bloody diarrhea, intestinal cramping, headache, fever, and malaise. The Emergency Department, <\/span>
various primary care offices, and several urgent care offices reported symptoms for this outbreak <\/span>
to the Springfield Health Department who launched an investigation using Epi Info 7.2. <\/span>
You are responsible to create an outbreak survey, using Epi Info 7.2 with the following <\/span>
questions:<\/span>

2.<\/span> Which food(s) did you eat? Did you eat leftovers the next day after the slumber party?<\/span>
3.<\/span> When did the symptoms start?<\/span>
4.<\/span> What were the symptoms you experienced?<\/span>
